Log in with Registration Number & Date of Birth

Enter the Reason for Name of Correction.

3.

Upload the Original certificate issued by Telangana Pharmacy Council.

4.

Upload the letter addressed to The Registrar requesting for Genuineness certificate.

5.

Complete the payment through online.

Note :     Name Correction Fee is Rs.50/- + 18% GST.
